Start the accommodation process by completing the student self-disclosure form.

Students who have previously had an IEP or 504 plan during high school AND/OR who have a documented disability (e.g., Diabetes, Crohns, ADHD, Autism, Mental Health diagnosis) may be eligible for accommodations.
