Applying for Health Insurance During OPT

 

To apply for health insurance during an approved OPT period, a student must submit two separate E-Forms:

  • OPT Health Insurance Application E-Form;
  • Verification of International Health Insurance Fee Payment E-Form.

Here are the instructions for submitting the OPT Health Insurance Application e-form:

  1. Login to Sunapsis.  Click here for instructions.
  2. After logging in, click on ‘Insurance and Finances’ in the shaded box on the left side of the screen, then click on ‘OPT Health Insurance Application’.  Finally, click on the ‘Start a New Request’ prompt.  Please note:  Any OPT Health Insurance applications you previously submitted will be displayed on the page (oldest first):OPT Health Insurance Screenshot
  3. The page will then display the two E-Forms necessary for completing the OPT Health Insurance process.  The selection box for the verification E-Form will be grayed out – you cannot submit this E-Form until the initial E-Form has been approved by the International Health Insurance team.  Click on the ‘OPT Health Insurance Application’ prompt:OPT Health Insurance Application Screenshot
  4. The E-Form lists the US Address currently on-file with ISSS.  If this address is incorrect, please submit the ‘OPT Address Update’ E-Form.  You must provide the start and end dates for your insurance coverage, and the number of months.  Please note: 
    1. Start Date is ALWAYS the 15th of the month;
    2. End Date is ALWAYS the 14th of the month.
  5. Read the statement in the confirmation box CAREFULLY.  You must click on the box to signify you have read and understand the statement.
  6. When complete, click on the ‘Submit’ button at the bottom of the form:OPT Health Insurance
  7. The ‘OPT Health Insurance’ page is displayed.  The box next to the ‘OPT Health Insurance Application’ prompt shows the E-Form is pending review.  A date/time stamp indicates when the E-Form was submitted for processing, and the start and end dates entered.  You may process other E-Forms, or log out of Sunapsis by clicking on the button in the shaded area on the left side of the page:OPT Health Insurance Final Screenshot

You will receive an email after the ISSS Health Insurance Team has completed processing the E-Form.  If it is approved, the subject of the email will be “E-Form Approved:  OPT Health Insurance Application”.  In order to complete the enrollment process, you MUST pay your International Health Insurance Fee.  Click here for more information.

 

Here are the instructions for submitting the Verify OPT Health Insurance Payment E-Form:

  1. Login to Sunapsis.  Click on ‘Insurance and Finances’ in the shaded box on the left side of the screen, then click on ‘OPT Health Insurance Application’.  Finally, click on the prompt for the request currently being processed:OPT Health Insurance Payment Start
  2. Note that the ‘OPT Health Insurance Application’ is flagged as ‘Complete’.  Click on the ‘Verify OPT Health Insurance Payment’ prompt:OPT Health Insurance Payment Main
  3. Check the box to signify you have paid the International Health Insurance Fee, then click on the ‘Submit’ button.  Please Note – do NOT submit this E-Form until AFTER you have paid the International Health Insurance Fee:Verify OPT Health Insurance Payment Process
  4. The ‘OPT Health Insurance’ page is displayed, and the box next to the ‘Verify OPT Health Insurance Payment’ prompt indicates the E-Form is pending review.  You may process other E-Forms, or log out of Sunapsis by clicking on the button in the shaded area on the left side of the page:Verify OPT Health Insurance Payment Process
  5. You will receive an email once the International Health Insurance team has completed the enrollment process.
  6. If your Verify OPT Health Insurance Payment E-Form is denied (indicated by ‘X’ inside a red box), PAY THE FEE!  Once this is done, you can submit a new E-Form by clicking on the ‘Add New Verify OPT Health Insurance Payment’ prompt:Verify OPT Health Insurance Payment Process-denied
