Template 1: initial contact with the professor
Email template to schedule an initial meeting with the professor to discuss the implementation of your accommodations
Dear Professor Smith,
My name is XXX and I am a student in your xxx course. You have received a copy of
my Academic Accommodation Letter. I would like to set up a time to discuss how I will
access my authorized accommodations as a registered student with Services for Students
with Disabilities. Would you prefer I stop by during your scheduled office hours,
or would a separate meeting be more appropriate, given the nature of our conversation?
I look forward to hearing from you and working together to facilitate these accommodations.
Thank you in advance for your assistance.
Best Regards,
Template 2: follow-up with professor
Email template to the professor after your initial meeting if you have not received instructions on how your exam accommodations will be implemented.
Dear Professor Smith,
I hope this email finds you doing well.
I am following up on the communication below in order to finalize arrangements for
my approved exam accommodations in your course. For clarity and ease of reading, I've
put my questions in bullet format. If it would be helpful for us to meet in person
instead, I would be happy to do so. Please let me know which works best for you.
- Question #1 – Where will the test take place?
- Question #2 – Will this be where I get my extended time?
- Question #3 – Who will the proctor be for the exam?
