- Time Off Requests
When you plan to take time off, you should obtain a “Request for Time Off” form from an Administrative Support staff member. Fill it out in entirety and submit to your supervisor for approval. Upon approval forward the white copy to Hope Reese for attendance reporting purposes. You should then post your Google calendar accordingly. The time should be entered as a meeting on your own calendar so that others recognize a conflict. - Absenteeism Call-In Policy
All employees with an 8:30 a.m. start time must call in by 8:00 a.m. (7:30 a.m. during summer hours) to report an unscheduled absence. Before 7:00 a.m., you should call the Monitor Board (x7-2341). After 7:00 a.m., you can reach the Customer Service Center (x7-2226). CSC/Monitor Board will then pass the list of call-ins along to the Administrative Support Team by 8:30 a.m. (8:00 during summer hours) for inclusion on the daily attendance report. An absentee report, including the call time and reason for absence will also be sent to the employee's supervisor. - Time and Attendance Recording/Forms/Accrual
